GENERAL INFORMATION FROM YOUR SCHOOL NURSE

The primary mission of the School Health Service department at Waukegan Community Unit School District #60 is to promote the health and safety of students, recognizing that healthy children are better learners. School Health Services, provided by a School Nurse, are available to all students at Abbott Middle School. If you would like to discuss School Health Services related to your child's healthcare needs, please contact me.

PHYSICAL EXAM AND IMMUNIZATION REQUIREMENTS FOR MIDDLE SCHOOL STUDENTS

All students entering 6th grade must have a Physical examination by a licensed health care provider within a year of the start of 6th grade. For the Physical to be considered complete, parents must complete the Health History section of the Physical. When a student enrolls at TJ in a grade other than 6th, the entry Physical must be dated within a year of the start of their 6th grade year. The next Physical exam requirement is when a student enters 9th grade.

Immunizations (IZ) must be up to date. I have included links to the Center for Disease Control (CDC) website for information on childhood vaccines. The schedules are for vaccines from birth to 6 years of age and for Preteens and Teens. If you have any questions about your child's IZ requirements, please contact me.

If you claim a religious exemption for your child's immunizations, it must be discussed with your child's Provider. Indicate your religious exemption on the linked Certificate of Religious Exemption form and after discussing it with your child's Provider, who then signs the form, submit it to the School Nurse who will submit it to the Superintendent for approval.

SPORTS PHYSICAL

All students participating in a sport in Middle School must submit a Sports Physical or a School Physical completed by a licensed Provider dated within 395 days of the sport in which they are to participate. The parent must complete the parent side of the Sports Physical. The Spanish version of the parent side of the Sports Physical is a separate link.

DENTAL EXAM

All 6th Grade students must submit a Dental Exam dated between 11/15/16 - 5/15/18.

EYE EXAM

An Eye Exam is required for any new student entering the Illinois school system for the first time, regardless of grade level.

IF YOUR CHILD TAKES MEDICATION AT SCHOOL

Please see the attached Regulations for Prescribed Medication (E/S). The Authorization of the Administration/Self-Administration of Medication form must be completed by your child's Provider and signed by you. All medication must be brought to school in the prescription/original bottle.

IF YOUR CHILD HAS ANY CHRONIC HEALTH CONDITION, PLEASE CONTACT ME. Below are forms you may need.

  • ASTHMA: If your child is treated for Asthma please submit an Asthma Action Plan (AAP) and a Medication Administration form (linked above) if your child needs a rescue inhaler at school.

  • DIABETES: If your child has Diabetes, your child's Provider will give you a Diabetes Management Plan (DMP) to help manage his/her Diabetes at school. Please submit the Plan along with the Diabetes supplies to the Nurse.
  • SEIZURE DISORDER: If your child has a Seizure Disorder, please submit a Seizure Action Plan completed by your child's Provider and contact me if your child needs to have rescue medication at school.
